Our facilities include a state-of-the-art Behavioral Health Center, Cancer Center, Chest Pain Center w/PCI-Resuscitation, Family Center, Heart & Vascular Center, Imaging Services, Neonatal Intensive Care Unit, Orthopedic/Spine Center, Physical Therapy Services, Rehabilitation Center, Stroke Center, Surgical Services, Surgical Weight Loss Center, Network-of-Care Affiliated Physician Practices, and much more. The case management assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges.
