Coordinate care with specialists, hospitals, behavioral health providers, dental providers, schools, community organizations, and other healthcare professionals, including referral or transfer to a higher level of care when appropriate. Responsibilities: Provide comprehensive outpatient pediatric care, including preventive and well-child visits, acute care, chronic disease management, developmental assessments, behavioral health screening, diagnostic evaluation, treatment planning, and follow-up care.
