The medication historian technician will use various resources to gather this information including calling physician offices, dialysis centers, pharmacies, hospitals and nursing homes and obtaining medication administration records to confirm outpatient regimens. Ability to interact with patients and their caregivers as well as various members of the healthcare team (i.e., nurses, pharmacists, physicians, outpatient providers, nursing home staff).
