In this role, you will serve as a trusted clinical resource for patients throughout their home health journey, providing skilled nursing care, coordinating services, educating patients and families, and helping individuals safely manage their health needs outside of the hospital. Essential Functions: Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient's needs are met and quality care provided in accordance with Federal, State, and program guidelines.
