· Develops and monitors behavioral intervention programs in consultation with professional and direct care staff for assigned residents.
· The individual’s primary purpose in this position is to implement social service interventions that address the psychosocial needs and enhance the quality of life for residents and/or their responsible parties while linking social supports for the physical, mental, and financial well-being of residents in, and post discharge from the facility.
