The proposed Medical Social Worker (SW) will support patients with hepatitis C (HCV) in accessing and navigating care by identifying psychosocial and practical barriers, conducting social assessments, providing counseling, and connecting patients and their families with available national, state, institutional, community, and pharmaceutical resources. Coordinating referrals and appointments, completing Medication Assistance Program (MAP) and prior authorization (PA) paperwork, facilitating insurance and medication access, assisting with financial and social needs, and conducting follow-up outreach to promote treatment initiation, completion, and confirmation of sustained virologic response (SVR).
