The packet includes the admission agreement, resident rights, notification procedures, medical consent form and safety proceduresOrients the resident/patient and significant others to the facility, addressing any needs or concerns regarding the resident/patient adjustmentProvides social work assessments and counseling to assist residents/patients and significant others in their adjustment to the new environmentCompletes resident/patient assessments and as a member of the ICCP team develops aInterdisciplinary Comprehensive Care Plan for each individualAssesses resident/patient status including strengths, coping mechanisms, personal goals, progress towards goals, significant relationships, and problem areasDevelops a plan with the ICCP team that addresses the resident's/patient's problem areas and goalsEnsures that resident/patient and significant others are involved in the care plan processEstablishes and maintains an on-going relationship with residents/patients and significant others.
