Home Health RNs assess patient conditions, develop care plans, administer treatments, and educate patients and caregivers on disease management.Essential Work Functions:<ul><li>Assess patient conditions and develop individualized care plans based on medical needs and physician directives</li><li>Administer medications, treatments, and wound care while monitoring patient responses</li><li>Provide patient and caregiver education on medication management, disease progression, and self-care techniques</li><li>Monitor for changes in patient status, reporting concerns and adjusting care plans as needed</li><li>Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care transitions</li><li>Document patient care, interventions, and progress accurately and in compliance with regulatory standards</li><li>Ensure compliance with infection control, safety, and home care best practices to protect patients and caregivers</li><li>Support families and caregivers, offering guidance and emotional support throughout the care process</li><li>Maintain communication with case managers and interdisciplinary teams to provide coordinated patient care</li><li>Complete charting and documentation requirements while meeting visit and productivity goals</li><li>Adhere to infection control, safety, and healthcare regulations</li><li>Perform other duties as assigned within the scope of practice</li></ul>Required Essential Skills:<ul><li>Critical thinking, service excellence, and good interpersonal communication skills</li><li>The ability to read, write, and communicate in the English language</li><li>Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills</li><li>Physical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bend</li><li>Interpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism</li><li>Technical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipment</li><li>Mental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure.
