The LMHP is responsible for accurately documenting services, participating in Child & Family Team meetings as needed, and communicating participant progress to supervisors, team members, and the referring authority. Services may be provided in the participant's home or appropriate community settings, or in the YAP office, and may include formal assessments, service planning, and individual, family, and group counseling.
