What You’ll Do:• Deliver care based on Individualized Service Plans (ISP)• Assist with ADLs including bathing, dressing, mobility, dining, and toileting• Obtain and report vital signs, intake, and changes in condition• Partner with licensed nurses on restorative and care planning programs• Support fall prevention, skin care, and quality improvement initiatives• Maintain accurate documentation and restorative records• Respond promptly to resident, family, and visitor concerns• Follow infection control, OSHA, and safety guidelines at all timesYou play a critical role in promoting dignity, safety, and quality of life for every resident you serve.
