Responsibilities: Community Outreach & Partnership Development Develop, strengthen, and maintain collaborative partnerships with community organizations, veteran-serving agencies, healthcare providers, schools, military-affiliated organizations, housing agencies, peer-support programs, and social service networks. The Case Manager actively develops and maintains collaborative relationships with community organizations, veteran-serving agencies, healthcare systems, schools, social service providers, and military-connected networks to strengthen referral pathways and improve access to care.
