Provides daily, direct, and indirect services to assigned caseload; meeting with the person served where they are at in order to assist the individual with daily living skills, educating them regarding diagnosis and treatment, accessing services and resources as needed, including financial assistance, medical assistance, housing assistance, crisis services, and meeting their immediate needs of food, clothing, and shelter.
