Performs under the direct supervision of the Nurse Manager to ensure quality care during daily in-person visits to patients receiving acute care at home through Home Hospital which includes daily assessments, vital sign monitoring, and patient education; lab draws, wound care, and medication administration may also be necessary and will be closely coordinated with the Home Hospital nursing and provider teams. Provides quality acute/inpatient level of virtual support to the Home Hospital healthcare team if needed including triage of calls, virtual medication administration, virtual visits, telephone outreaches, education, and escalation of services both clinical and non-clinical.
