Documentation & Communication:• Accurately document outcomes related to resident care, medication administration, behavior, activity, and culinary services.• Communicate changes in condition, behavior, or care needs promptly to the nursing leadership team.• This position is responsible for administering medications and assisting with daily living activities while ensuring care is delivered with compassion, dignity, and respect for each resident in Assisted Living and Memory Care.
