The OAC will assist as needed with discharge, aftercare consultation, and technical assistance for older adults with serious mental illness (SMI) and or substance use disorder (SUD) who are or have been discharged from local hospitals, state psychiatric hospitals, residential SUD treatment programs and homeless shelters to less restrictive environments (Recovery Residences, assisted living facilities, nursing facilities, and family placements) in Baltimore City. Provide case consultation, technical assistance, training, and overdose prevention support to: Community residential settings (including but not limited to families, Recovery Residences, Residential Rehabilitation Programs, Assisted Living Facilities, Nursing Homes, shelters, Baltimore City Senior Centers and Adult Medical Day programs.
