Serves as a leader, change agent and coordinator of patient care services; takes a lead role in overall design and implementation of a patient centered care delivery system that facilitates achievement of patient outcomes, supports continuity of care and enhances patient and care giver satisfaction. Exact salary will ultimately depend on multiple factors, which may include the successful candidate's geographical location, skills, work experience, market conditions, internal equity, responsibility factor and span of control, education/ training and other qualifications.
