Interfaces with other healthcare team members for appropriate referrals/services (ethics, social work, fitness center, pastoral care, patient advocate, financial counseling, care management, community services, dietary, palliative care, speech therapy, etc.). Follows patient through the entire care continuum/experience, eliminating operational barriers to services and addressing or making referral to address all other barriers to services or care compliance with particular focus on disparate populations.
