The Nurse Practitioner NP practices independently and in collaboration with appropriate physicians to provide primary care to patients and oversees, conducts and/or completed all required visits and documentation in accordance with CMS hospice regulations for face-to-face encounter visits. Assess and prioritize patient needs, place orders and communicate with medical staff, nursing staff, social services, therapy, dietary, consultants, referring, and primary care physicians as well as the hospice Medical Director to coordinate patient's plan of care.
