Overview: The Value-Based Care (VBC) Nurse Practitioner provides patient-centered clinical care and care management for individuals with kidney disease, collaborating with nephrologists, dialysis care teams, and other providers to support quality outcomes, reduce hospitalization risk, and promote effective management of chronic and comorbid conditions. Primary duties include: Holistic patient care management including but not limited to engaging patients, screening for depression, addressing risks of hospitalization, providing transitions of care following hospitalizations, and supporting management of kidney health and comorbid conditions.
