You would be joining a team of family physicians, nurse practitioners, RNs, pediatricians, OB-GYNs, dentists, specialists (optometrist, podiatrist, dermatologist), licensed social workers, mental health professionals, case managers, health educators, registered dietitians, LVNs, and promotoras to mitigate social determinants of health. Our health centers use a team-based model of care that includes primary care, family medicine, obstetrics, womens health, oral and vision care, preventative and educational health, mental health, patient advocacy, case management, specialty care, and community health.
