Not currently have any professional disciplinary proceeding of any state or federal authorities or any disciplinary action of any medical facility in any jurisdiction Has never been excluded, suspended, or otherwise sanctioned by Medicare, Medicaid, or any other third-party payor, nor listed on any state or government list of excluded individuals, parties, or specially designated nationals list as debarred, excluded, or otherwise ineligible for participation in government healthcare programs. A healthcare provider must hold a valid and in-good-standing license and/or certification to practice in the State or Country where the facility of employment is located; or be eligible to obtain such a license and have current CPR/AED/BLS certification.
