Essential Functions: Assists in the admission process of the client to the Agency by performing an initial evaluation, assessing the client's psychosocial, financial, environmental and community needs as evidenced by POC, documentation, clinical records, conferences, and community resource referrals. Provides effective communication to clients, their families, team members, and other health care professionals to achieve desirable outcomes, and regular participation in Case Conference/Team Meetings.
