In this temp-to-perm, field-based role, you will provide community nursing, home visits, care coordination, discharge planning, medication management, and clinical support for individuals with intellectual and developmental disabilities (IDD) and behavioral health needs. Responsibilities as the Community Registered Nurse (RN): Community Assessments: Conduct nursing assessments for prospective admissions, complete home visits, and evaluate individuals transitioning from hospitals into community-based programs.
