The CBHC (Community Behavioral Health Center) Registered Nurse will perform the following core functions: complete triage evaluations, administer injectable medication, engage in medication reconciliation, take vital signs, perform phlebotomy, coordinate medical care, contribute to comprehensive assessments, complete individual crisis prevention planning, provide psychoeducation, and assist with transition of care planning and care coordination. Support individuals in participating in other CBHC services, E.g., Peer Support, Recovery Coaching, Care Management, Psychiatric Rehabilitation Services, Social Support Opportunities, and Community Recovery Supports.
