The Non-Routine Placement Nurse serves as a clinical liaison for complex and high-risk cases, reviewing clinical information, identifying appropriate care settings, resolving placement barriers, and coordinating with internal and external care teams. Work collaboratively with case managers, social workers, physicians, hospitals, physician groups, and post-acute care providers to identify appropriate care options and facilitate successful transitions.
