p>Under the general supervision/direction of the Acute Rehabilitation Nurse Manager, and in collaboration with the treatment team and external entities, the Rehabilitation Admissions Nurse shall monitor, coordinate and facilitate the interdisciplinary plan of care, effective clinical resource management, discharge planning, and continuation of post acute care needs of patients assigned.
