Providing Person-/Population-Centered Nutrition Care: The registered dietitian nutritionist (RDN) conducts nutrition care process and workflow elements to identify and address nutrition-related problems which a RDN is responsible for treating, incorporating the following elements:Reviews or obtains nutrition screening data to identify malnutrition or risk of malnutritionObtains and evaluates medical, nutrition, and food-related information for relevance and accuracyIdentifies and labels nutrition problem(s)/diagnosis(es)Develops plan and implements culturally appropriate person-/population-centered nutrition interventionsMonitors and evaluates person-/intervention-specific indicators and outcomes data to determine whether planned interventions should be continued or revisedDocuments and communicates results with interprofessional team and patients/clients/caregivers.
