Key Responsibilities • Evaluate patients' functional abilities, mobility deficits, and rehabilitation needs upon admission and throughout the care continuum • Develop and implement individualized physical therapy treatment plans aligned with each patient's goals and physician orders • Perform therapeutic interventions including therapeutic exercise, manual therapy, gait training, and neuromuscular re-education • Monitor and document patient progress, adjusting treatment plans as clinically indicated • Educate patients and family members on home exercise programs, safe mobility techniques, and fall prevention strategies • Collaborate with the interdisciplinary rehabilitation team including occupational therapists, speech therapists, nurses, and case managers • Participate in discharge planning to ensure safe transitions and appropriate follow-up care ✅ Requirements • Active VT PT license • BLS/BCLS certification • 1+ years of Rehabilitation Physical Therapist (PT) experience • Graduate of an accredited Physical Therapy program with a Doctor of Physical Therapy (DPT) or equivalent degree • National Provider Identifier (NPI) number Pay Pay range: $2,124 to $2,196 per week.
