After Hours Hospice Triage RN will work a combination of ONSITE Call Center Triage RN hours in the Bronx NY office (Registered Nurse MUST live in the NYC metro) and will also do emergency hospice patient visits when needed in the community, usually to a SNF, hospital and sometimes to the home of the hospice patient. Hospice Triage RN provides patient physical and psychosocial health systems assessments telephonically, supporting the Plan of Care, “Goals of Care” based on the patient’s and caretaker’s needs/wishes, and in coordination with plan developed/placed by the primary RN Case Manager for the patient.
