Implement intentional strategies to identify at-risk students and monitor students' progress through regular email, text messaging, and phone, and through scheduled meetings or proactive outreach and communication, responding to students' questions, concerns, and needs with compassion, respect, encouragement, and empathy. Identify barriers to student success and coordinate referrals to appropriate college and community resources, such as Academic Advisors, Career Development, Educational Support Services, Student Life, Counselors, Community Resources, Financial Aid, faculty, and others to address a range of student needs.
