You'll review patient information to identify missing preventive or chronic care needs, conduct patient outreach, support Annual Wellness Visits, coordinate screenings and follow-up care, and work with primary care teams to close important care gaps. • This position focuses on Medicare Advantage, preventive care, chronic disease management, Annual Wellness Visits, patient outreach, and closing gaps in care across a large outpatient patient population.
