Collaborates with physicians, nursing, and the interdisciplinary team to coordinate treatment and contribute to discharge planning by assessing functional readiness for discharge, recommending equipment and home modifications, and communicating anticipated discharge needs to support safe transitions and continuity of care. Additionally, St. John's is proud to be redesignated as a Baby-Friendly Hospital by Baby-Friendly USA - the accrediting body and national authority for the Baby-Friendly Hospital Initiative (BFHI) in the United States.
