This position works closely with patients, families, physicians, facilities, and the hospice interdisciplinary team to manage symptoms, support goals of care, and help patients remain as comfortable as possible throughout the end-of-life journey. Coordinate care with physicians, facilities, pharmacies, medical equipment providers, contracted vendors, and other members of the care team.
