The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes in accordance with the patient's established plan of care, physician/provider orders, Pennsylvania nursing laws and regulations, applicable federal requirements, and agency policies and procedures. The LPN works collaboratively with the RN, physician/provider, patient, family/caregivers, and interdisciplinary team to promote patient safety, continuity of care, functional independence, and appropriate self-care.
