Beyond individual cases, you'll help develop Veteran-specific care coordination workflows, identify opportunities to improve the program, and work with care coordination and training leadership to strengthen the team's understanding of VA healthcare, benefits navigation, Veteran resources, and common barriers to care. You'll serve as a resource to other care coordinators, help improve Veteran-specific workflows, identify gaps in the program, contribute to training, and help shape how care coordination is delivered as the program develops.
