As an Intensive Case Manager, you will assist adults with mental illness gain access to needed resources such as medical, social, educational, and other services with outcomes focused on independent living, vocational and educational participation, adequate social supports, and reduce hospitalization. High school diploma with twelve (12) credits in sociology, social welfare, psychology, gerontology, or other social sciences and two (2) years of experience in human services with one (1) year of direct client contact.
