The MGBfC Community Health Worker is embedded within pediatric primary care, collaborating closely with the multidisciplinary primary care team in order to support families with basic care coordination, assessment of unmet health-related social needs, and navigation to appropriate hospital and community resources to address identified needs. CHWs use their unique skills (knowledge of community resources, motivational interviewing, trauma sensitive care, health coaching, etc.) to help patients coordinate their healthcare, address unmet health-related social needs, and gain confidence in managing their family’s health needs.

The CHW III will participate in the development and implementation of innovative, cost-effective, and quality improvement delivery system changes that proactively meet the complex needs of our patients, including efforts to manage their behavioral and social needs in addition to their medical care. The CHW III partners closely with different providers, patients, and outside organizations and is responsible for facilitating communication between patient and care teams across the care continuum and providing culturally appropriate health education and self-management support.

Inpatient Social Workers work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.

The per diem social worker works collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community, and assesses and arranges discharge planning needs and post hospital follow up disposition.

em> Excellent knowledge of community resources for behavioral health and other social service needs The ability to create and implement integrated care plans (if needed), ensuring access to an array of linked services, and the exchange of information among patients, family members, and providers Maintains an understanding of the transient and chaotic nature of the lives of patients experiencing homelessness, carceral involvement, mental health disorders, and substance use disorders, and adjusts approach to offering treatment by meeting patients where they are at Demonstrated ability to work effectively with people from diverse cultural and linguistic backgrounds; bilingual/multilingual skills are valued (e.g., Cape Verdean Creole, Portuguese, Haitian Creole, French, Spanish Return patient and staff communications in a timely and appropriate manner Appropriately document patient care in the electronic medical record May be assigned to support various Substance Use Services programs and service sites (e.g., clinic-based, community-based, and/or mobile/field-based services) based on department needs Other duties as assigned REQUIREMENTS: Bachelor's degree in Social Work or related field preferred Local travel as necessary. This position builds trusting relationships with individuals who experience barriers to care, delivers harm reduction and health education, and supports connection to treatment, recovery supports, primary care, behavioral health, housing, and other community resources, with a commitment to cultural sensitivity and trauma-informed practice.

In addition, this role focuses on performing the following Social Work duties: Provides social work assistance to patients and families relating to illness, disease, hospitalization and life events in all areas of service as a member of multi-disciplinary team and in liaison with community resources.

The Community Health Worker (CHW)/Patient Navigator plays a key role in delivering outreach, navigation, and support services to patients of Worcester Health Care for the Homeless, which is located at 199 Chandler Street, Worcester. This position focuses on building trusting relationships with individuals experiencing homelessness, connecting them to medical and behavioral health care, and supporting their progress toward stability and improved overall well-being.

p>The Clinical Social Worker (LCSW) is expected to demonstrate the following:

• An understanding of, and comfort working with patients of all ages who suffer complex medical and psychiatric problems; the ability to work with the families of such patients, and the ability to help such patients and families understand and access the resources required to support care. The Neuro-recovery Clinic supports the recovery and reintegration of patients who have sustained severe neurological injuries (e.g., major stroke or brain bleeding, aneurysms, brain injuries) that have led to significant disabilities.
