Performs home safety inspection, patient assessment, medication reconciliation, point-of-care testing, telehealth facilitation, and equipment setup to support safe transition of care and reduce readmissions. Training / Certification / Licensure: Current New Jersey paramedic certification issued by the NJ Department of Health Current BLS CPR certification (American Heart Association or equivalent).
