p>JOB DESCRIPTION: Responsible for coordinating and completing referrals ordered by providers, acquiring and verifying third party information for referrals, obtaining and arranging referral appointments, and/or tracking referrals and task as deemed necessary. Make appropriate notations on patient's health record and bring to the attention of the ordering provider any problems regarding obtaining referrals including:
Obtaining authorizations from third party payors.
ul>Assists providers with clinical procedures such as taking vital signs, wound care, suture removal, administering injections, phlebotomy, vaccines, performing urine tests, EKGs, and applying splints or other specialty-specific tasks. This role supports clinical and administrative functions, including assisting with medical procedures, maintaining exam rooms, managing patient intake, and ensuring efficient clinic operations while delivering excellent patient service.
p>Bachelors Degree in Nursing, Business, Health Administration, or related field required Masters Degree in Nursing, Business, Health Administration, or related field preferred 2-4 years of experience in patient experience, quality improvement, or a related healthcare role required 1-2 years of leadership experience preferred. The Community Cares Champion serves as the primary point of contact for the facilitys patient experience initiatives, including HCAHPS performance, and provides coaching, training, and tools to achieve high-quality outcomes.

This position serves as a liaison between patients, families, physician offices, surgeons, proceduralists, scheduling, pre-assessment, anesthesia, and community resources to support safe, timely, and organized care before, during, and after procedures Essential Job Functions: Serves as the primary communication link between Surgical Services and physician offices to facilitate case scheduling, case readiness, patient instructions, required documentation, and timely resolution of barriers that may delay procedures. BSN required ; MSN preferred BLS certification At least 1-2 years experience in oncology nursing; National Certification in Oncology preferred Oncology Nurse Navigator Certification required or obtained within 1 year Minimum of two years of clinical nursing experience required.

Non-US Accredited Foreign Graduates are required to successfully complete their internship within 18 months and will be skilled in diverse patient care, practice management, leadership, and education, and will be prepared to complete and pass all required Pharmacist licensure exams and processes within 120-days of the completion of required US Internship hours. Deliver additional health care services as allowable by state regulation, individual certification, and as directed by pharmacy leadership team, including but not limited to point-of-care and/or swab-and-send testing, immunization preparation and/or delivery, etc.
