If not practicing under a collaborative practice agreement or an approved protocol, contact providers when there appears to be a possibility of unsafe or inappropriate medication use due to factors including, but not limited to: allergy, medical condition, drug interactions, dosing, and methods of accurately documenting all clinical activities and interventions in the electronic record. Engages patients and healthcare professionals to efficiently and effectively maximize therapeutic outcomes and is responsible for one or more of the following tasks: dispenses medications, monitors patients' drug therapies, works under a collaborative practice agreement or per protocol, and provides consultation and education to patients and other staff.
