Implements the plan developed by the Social Worker that addresses the patient''s identified needs, issues, resources, and care goals; determining the choices available to individual patients; and educating the patient/family on available options; prioritizing patients according to intensity, need, and required follow-up. EDUCATION AND EXPERIENCE: Required Qualifications: A bachelor''s degree in social work, human services, psychology, public health, criminal justice, sociology, anthropology, or related field.
