This position will help individuals deal with complex and comorbid diagnosis which may include: serious persistent mental health illness, addictions and substance abuse; family, parenting, marital problems and challenges with social determinants of health; suicide; stress management; problems with self-esteem; and issues associated with aging, physical, mental and emotional health. Collaborate with other staff members, primary care practice partners including pediatricians, doctors, nurse practitioners, office staff, and referral sources as assigned to perform clinical assessments and develop treatment plans.
