Knowledge of acute care, subacute care, long-term care, home care, hospice interventions, rehabilitation options, County mental health services, and other community resources and requirements- strongly preferred. This role supports patients with complex psychosocial needs and collaborates with interdisciplinary teams to ensure effective care, improved outcomes, and safe transitions.
