Responsible for providing quality pharmaceutical care to patients of Valley Medical Center Hospital, including ensuring appropriate drug therapy selection, including indication(s), dose, route, drug interactions, drug allergies and ensuing accurate order entry/verification and dispensing of drug therapy (as applicable). Responsibilities include, but are not limited to direct patient care, medication therapy management, drug therapy consultation via collaborative drug therapy agreements (CDTAs) drug dispensing, participation in transitions of care, and provision of patient education and counseling for assigned patient care areas.
