Performs Physical Therapy (PT) evaluation, reassessment and consultation; provides physical therapy plan of care, goals, discharge planning, patient and significant other (SO) education; renders physical therapy services, interventions, use of therapeutic modalities within the practice guidelines of the American Physical Therapy Association (APTA) and other regulatory agencies; and participates in collaborative patient care services. Mature, resourceful; commitment to “Patient First” and safe, quality patient care; team player, personable and dynamic with the ability to work in a diverse cultural setting.
