The TOC Pharmacist collaborates with physicians, nurses, case management, and outpatient pharmacy teams, participates in departmental and interdisciplinary initiatives, and contributes to systemwide efforts to reduce medication‑related harm and improve transitions‑of‑care outcomes. The TOC Pharmacist provides comprehensive pharmaceutical care with a focus on accurate medication reconciliation, evidence‑based medication management, and safe transitions between inpatient and outpatient settings.
