Job Summary: Responsible for case managing patients enrolled in the cardiac and pulmonary rehabilitation programs, for providing wellness and fitness related services to the public as well as EvergreenHealth employees. Identifies and facilitates referrals to other healthcare providers (i.e., Psychiatrist, Dietician, Vocational Counselor, etc.) as determined by the patient's treatment plan.
