Communicate identified patient needs for additional home health services, including Home Health Aide (HHA), Occupational Therapy (OT), Physical Therapy (PT), Medical Social Work (MSW), Speech Therapy (ST), and dietary services, to the RN or Clinical Supervisor/Nursing Supervisor. In this role, you will implement the established plan of care, provide prescribed treatments, document patient care accurately, educate patients and caregivers, and assist with specialized procedures as directed.
