As a Clinical Nurse II on the 7 HV Cardiac Medicine Unit, you will: • Provide comprehensive, evidence-based nursing care to adult and geriatric patients with complex cardiac conditions • Assess, monitor, and manage patient status, including cardiac rhythms, hemodynamics, and response to treatment • Care for patients pre- and post-cardiac and vascular procedures, as well as those admitted for medical cardiac conditions • Administer medications and treatments safely, including cardiac drips and therapies, in accordance with policies and protocols • Collaborate closely with physicians, APPs, pharmacists, and interdisciplinary team members to develop and implement individualized plans of care • Educate patients and families on disease processes, medications, lifestyle modifications, and discharge planning • Recognize and respond promptly to changes in patient condition, escalating care as appropriate • Document patient assessments, interventions, and outcomes accurately in the electronic health record (EHR) • Serve as a clinical resource and mentor to peers, contributing to unit-based education and quality initiatives. Care is delivered through a collaborative, interdisciplinary team model, including Clinical Nurses, APPs, physicians, pharmacists, care managers, chaplains, patient care technicians, unit secretaries, and specialized telemetry technicians in the Telemetry Command Center, who provide continuous rhythm surveillance and clinical support.
