ctively participates in rehabilitation team efforts to meet patient needs, including, but not limited to, Hand center case rounds, counseling and teaching patients and their families, discussing changes in treatment programs with MD's, coordinating health care plans with other involved health professionals, e.g., PM&R staff, nurses, social workers, prosthetics, and others, and assists in discharge planning as appropriate. M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
